TELEHEALTH APPOINTMENT INSTRUCTIONS
We are pleased you have chosen to schedule your child's telehealth eye examination. Please review the instructions below and let us know if you have any questions. Please note that many answers to your questions may be found in our TELEHEALTH FAQs section by clicking here.
THE DAY BEFORE YOUR APPOINTMENT

If you are a new patient, please complete our ONLINE NEW MEDICAL PATIENT FORMS by clicking here. Please email a photo of the FRONT AND BACK of your child's insurance card to registration@uceyecenter.com

All patients must complete the ONLINE TELEHEALTH CONSENT FORM by clicking here.

Please download the ZOOM application from the Apple App Store by clicking here, or from the Google Play Store by clicking here. This will be the platform in which you will video conference with our doctors.

Obtain your child's visual acuity. You can do this in one of two ways: print a wall chart or use a downloadable application.

Record your findings so you may discuss them with the doctor. If you were advised to send photos of your child, please be sure you have sent them.
THE DAY OF YOUR APPOINTMENT

Prior to your appointment, you will receive an email notification inviting you to the Zoom visit. It will have a unique meeting ID number as well as a passcode. Please log in at the time of your appointment.

Be sure to have a stable stand for your device, in an area with good natural lighting.

Position your child at eye level with the device in front of him/her. It is best if a young child is seated on a stable child seat or high chair.

Turn on your child's favorite animated show on a television several feet away so that they can stare off into the distance while facing the camera/device.

Try to have a small penlight or light available as it may be needed.
At the conclusion of the telehealth examination, our doctors will advise you as to when you should book your child's follow-up appointment. Please remember that these visits cannot replace a comprehensive eye exam, done in our office, often with the use of eye dilating drops and other equipment. However, the telehealth visit can help guide us as to when you must bring your child directly into the office. We thank you for you cooperation and assistance during this difficult time.
